PHI is protected health information, such as a patient’s name, member ID or claim number. Please don’t send it. For example, write “a CO-197 denial on a knee MRI” instead of pasting the remittance.

Corrections

A correction request is a note that tells us a page may be wrong or out of date. We check each one against the official source, such as the X12 CARC list (a CARC is the code a payer uses to say why a claim was paid differently) or the CMS manuals. If the page is wrong, we fix it and update its “Last reviewed” date. Our editorial policy explains how.

What we can and can’t help with

We can point you to the right guide or explain how a code or rule works in general. Topics we cover include:

  • Denial codes and appeals, such as CO-197
  • CPT and ICD-10 coding basics
  • Medicare and Medicaid billing rules
  • Prior authorization
  • Patient bills and insurance denials

We can’t review your own claim, contact your payer or give legal, compliance or medical advice.
