CO-206 Denial Code (CARC 206) 2026: NPI Missing on the Claim

CO-206 means the payer couldn’t find a required National Provider Identifier on the claim. One NPI field was blank. The remark code tells you which one, such as the billing provider, the rendering provider or the referring provider. Find the empty box, fill it with the right NPI and resubmit. In brief: CARC 206 reads “National Provider Identifier – missing.” Match the remark to the field, for example N257 for the billing provider in CMS-1500 Box 33a or UB-04 FL 56. Fill the NPI and fix the setting that left it blank.

Key terms

An NPI is the 10-digit National Provider Identifier that CMS issues to each provider and organization. A billing provider is the person or group that gets paid, and its NPI goes in Box 33a. A rendering provider is the clinician who did the service, reported in Box 24J. The CMS NPI page explains who needs one.

Example scenario (fictional names), part 1

CO-206 Denial Example: What Happened

Daniel Ortiz handles billing for Lakeview Family Medicine, which opened a second office in September. Dr. Priya Nair saw a patient there for a 99214 visit. The claim to Ridgeway Health Plan, a fictional payer, came back with CAS*CO*206 and remark LQ*HE*N257, “Missing/incomplete/invalid billing provider/supplier primary identifier.”

Daniel checked the claim. Box 33 had the new office address, but Box 33a was blank.

Why was only this office’s claim missing the group NPI? Keep reading, or jump to how the CO-206 denial was resolved.

Why denial code 206 appears

  • New location setup. For example, a new office added in the system without the group NPI.
  • New provider. A clinician added before their NPI was entered, so Box 24J goes out blank.
  • Referring or ordering provider. The name was entered, but the NPI field was left empty.
  • Facility claims. UB-04 FL 56 (billing NPI) or FL 76 (attending NPI) left blank.

How to fix a CO-206 denial

Missing NPI workflow: read the remark, find the blank box, look up NPPES, fix the profile and resend
  1. Read the remark. N257 is the billing provider. N290 is the rendering provider. N265 is the ordering provider.
  2. Find the blank field. Check Box 33a, 24J and 17b, or FL 56 and FL 76 on the UB-04.
  3. Confirm the NPI. Look it up in the NPPES NPI Registry so you enter the right one.
  4. Fix the setup. Add the NPI to the provider or location record so it fills every claim.
  5. Resubmit. Send a corrected claim (frequency code 7), or a new claim if the payer treated it as unprocessable.

Example scenario (fictional names), part 2

How the CO-206 Denial Was Resolved

Daniel found that the new office had been set up as its own billing location, and no one had linked the group NPI to it. Every claim from that office had gone out with Box 33a blank.

Fix: Box 33a now carries the Lakeview group NPI, Box 22 = 7 with the original claim number. Daniel linked the NPI to the location and pulled every claim from that office since the move.

Outcome: Ridgeway paid the 99214 claim and 11 others from the new office on the next cycle.

Names and details are illustrative; no real patient data.

CO-206 UB-04 claim form showing a blank FL 56 billing NPI filled on a 0137 replacement claim
Example (fictional data): on a UB-04 the billing provider NPI goes in FL 56; a blank FL 56 causes the same CO-206 denial.
  • Box 33a / FL 56: blank billing provider NPI filled with the group’s NPI (placeholder 1234567890).
  • Box 22 / FL 4 + FL 64: frequency code 7 with the original claim number (type of bill 0137).
  • Setup: the group NPI linked to the new office so every claim from it carries Box 33a.
