CO-207 Denial Code (CARC 207) 2026: NPI Invalid Format Fix

CO-207 means an NPI on the claim isn’t in a valid format. The number may be too short, too long or carry a wrong check digit. It’s usually a typo or a data entry problem, not an enrollment issue. Find the bad NPI, correct it and resubmit. (Primary source: X12 Claim Adjustment Reason Codes)

In brief: CARC 207 reads “National Provider identifier – Invalid format”. A valid NPI has 10 digits, and the last digit is a check digit. Fix the number in the box the remark points to, such as Box 24J for the rendering provider.

What makes an NPI format valid

An NPI is a 10-digit number with no letters. The check digit is the last digit, calculated from the first nine with the Luhn formula. A format error means the number fails that test, such as a dropped digit, an extra digit or two digits swapped. The CMS NPI standard sets this structure.

Example scenario (fictional names), part 1

CO-207 Denial Example: What Happened

Grace Holloway bills for Summit Physical Therapy. A new therapist, Ryan Cole, PT, started in August. His 97110 claims to Brightwater Health, a fictional payer, came back with CAS*CO*207 and remark LQ*HE*N290, “Missing/incomplete/invalid rendering provider primary identifier.”

Box 24J showed a 9-digit number. His intake form had the full number, so the problem was in the system.

How did a 9-digit NPI get into his profile? Keep reading, or jump to how the CO-207 denial was resolved.

Why reason code 207 appears

  • Dropped digit. For example, a 10-digit NPI typed as 9 digits.
  • Swapped digits. Two digits reversed, so the check digit fails.
  • Wrong number type. A tax ID, license number or Medicare PTAN entered in the NPI field.
  • Field length limits. An import or interface cut off the last digit.

How to fix a CO-207 denial

NPI format fix: read the remark, count to ten digits, copy from NPPES, correct the profile and resend
  1. Read the remark. N290 points to the rendering NPI. N257 points to the billing NPI.
  2. Count the digits. A valid NPI always has 10.
  3. Check NPPES. Look up the provider in the NPPES NPI Registry and copy the number.
  4. Fix the profile. Correct the NPI where it’s stored, not just on one claim.
  5. Resubmit. Send corrected claims (frequency code 7) for every affected date.

Example scenario (fictional names), part 2

How the CO-207 Denial Was Resolved

Grace found that the onboarding spreadsheet had a short column width. When Ryan’s profile was imported, the last digit was cut off. NPPES showed his full 10-digit number.

Fix: Box 24J now carries his full 10-digit NPI, Box 22 = 7 with the original claim number. Grace corrected his profile and checked the import file for other cut-off numbers.

Outcome: Brightwater paid all of Ryan’s 97110 claims on the next cycle. No other profiles had the error.

Names and details are illustrative; no real patient data.

CO-207 UB-04 claim form fixing a short attending provider NPI in FL 76 on a therapy bill
Example (fictional data): on a UB-04 a short NPI in FL 76 (attending) triggers the same CO-207 format denial.
  • Box 24J / FL 76: 9-digit 123456789 replaced with the full 10-digit NPI (placeholder 1234567893, which passes the check digit).
  • Box 22 / FL 4 + FL 64: frequency code 7 with the original claim number (type of bill 0137).
  • Box 24D / FL 44: 97110-GP with M25.561 (entered as M25561) is unchanged.

Stop CO-207 before claims go out

Many claim scrubbers can run the NPI check-digit test. Turn that edit on, so a bad NPI stops before the claim reaches the payer. Also check new provider profiles against NPPES on their first day.

Recap

CO-207 means an NPI failed the format check. Count the digits, copy the number from NPPES, fix the stored profile, and resubmit every affected claim.

FAQ

What does CO-207 mean?

An NPI on the claim isn’t in a valid format, such as the wrong length or a failed check digit.

How many digits does an NPI have?

Ten, and the last one is a check digit.

Can I bill the patient for CO-207?

No. With group code CO, the practice fixes and resubmits the claim.

Do I need to appeal?

Usually not. Correct the NPI and resubmit the claim.

Can a scrubber catch this?

Yes. Many scrubbers can run the check-digit test before the claim goes out.
