CO-208 Denial Code (CARC 208) 2026: NPI Not Matched Fix

CO-208 means the NPI on the claim is valid, but it doesn’t match what the payer has on file. The NPI may not be enrolled with that payer. Or it may be paired with the wrong tax ID or address. The fix is to make the claim and the payer’s enrollment record agree. (Primary source: X12 Claim Adjustment Reason Codes)

In brief: CARC 208 reads “National Provider Identifier – Not matched.” Read the remark, for example N516 for an NPI and EIN mismatch. Compare Box 25 and Box 33a with the payer’s file, then fix the claim or the enrollment.

Key terms

An EIN is the employer identification number, the tax ID a group bills under. It goes in Box 25 on the CMS-1500 and FL 5 on the UB-04. Enrollment means the payer’s record that links an NPI to a tax ID, address and contract. A mismatch is when the claim pairs those details differently from that record, such as an old tax ID after a merger.

Example scenario (fictional names), part 1

CO-208 Denial Example: What Happened

Olivia Grant manages billing for Harbor Orthopedic Group. In July, Harbor joined a larger group and got a new tax ID. Dr. Samuel Reyes’s 20610 claims to Pinecrest Health, a fictional payer, came back with CAS*CO*208 and remark LQ*HE*N516, “Records indicate a mismatch between the submitted NPI and EIN.”

Box 33a had the right group NPI. Box 25 still showed the old tax ID.

Was the claim wrong, or was the payer’s file out of date? Keep reading, or jump to how the CO-208 denial was resolved.

Why denial code 208 appears

  • Tax ID change. For example, a merger or a new legal entity, with claims still sent under the old EIN.
  • Enrollment not updated. The payer still links the NPI to the old tax ID or address.
  • Wrong NPI type. An individual NPI in the billing box where the group NPI belongs.
  • New provider not linked. The rendering NPI isn’t linked to the group in the payer’s records.

How to fix a CO-208 denial

NPI mismatch steps: read N516, compare claim and payer records, fix the stale side, resubmit claims
  1. Read the remark. N516 is an NPI and EIN mismatch. Other remarks point to the rendering or billing NPI.
  2. Compare the claim. Check Box 25, Box 33 and 33a against the payer’s enrollment record.
  3. Fix the claim side. If the claim used old data, update the billing setup.
  4. Fix the payer side. If the payer’s file is old, send an enrollment update. Medicare uses PECOS.
  5. Resubmit. Send corrected claims once both sides match.

Example scenario (fictional names), part 2

How the CO-208 Denial Was Resolved

Olivia checked with Pinecrest. Its file already showed the new tax ID for Harbor’s group NPI. The billing system was still pulling the old tax ID for claims dated after the merger.

Fix: Box 25 now carries the new EIN with the same group NPI in Box 33a, and Box 22 = 7 with the original claim number. Olivia set the new tax ID to apply to all claims dated after the merger.

Outcome: Pinecrest paid the 20610 claims on the next cycle. Olivia sent updates to two other payers whose files still had the old tax ID.

Names and details are illustrative; no real patient data.

Example (fictional data): fixing a CO-208 denial by sending the post-merger EIN in Box 25 with the same group NPI in Box 33a.

Example (fictional data): on a UB-04 the tax number in FL 5 must match the NPI in FL 56 the payer has on file.
  • Box 25 / FL 5: old tax ID 00-0000001 replaced with the post-merger EIN 00-0000002 (placeholders).
  • Box 33a / FL 56: group NPI unchanged (placeholder 1234567890).
  • Box 22 / FL 4 + FL 64: frequency code 7 with the original claim number (type of bill 0137).

Recap

CO-208 means the NPI didn’t match the payer’s records. Compare the NPI, tax ID and address on the claim with the enrollment file, fix whichever side is wrong, and resubmit.

Whoever has the wrong data, the practice for claim setup or the payer through an enrollment update.

Why did CO-208 start after a merger?
