M51 is a Remittance Advice Remark Code that means the procedure code on the claim line is missing, incomplete or invalid. It explains a reason code, most often CARC 16, and with Medicare that pair makes the claim unprocessable. (Primary sources: X12 Remittance Advice Remark Codes · CMS Medicare Claims Processing Manual, Ch. 1, §80.3 · Noridian JE Part B, CO-16/M51/N56)
Quick answer: M51 means the payer couldn’t accept the procedure code on the line. The code may be missing, deleted or invalid for the date of service, or an unlisted code may lack its description. With CO-16, you can’t bill the patient. Fix the code or narrative and send a new claim. Medicare gives these no appeal rights.

M51 remark code description (official X12 wording)
X12 Remittance Advice Remark Code M51, verbatim:
“Missing/incomplete/invalid procedure code(s).”
Source: X12 Remittance Advice Remark Codes, checked 6 October 2026. Start: 01/01/1997 | Last Modified: 12/02/2004. X12’s note on the code reads “(Modified 12/2/04) Related to N301.” N301 is “Missing/incomplete/invalid procedure date(s).”
A remark code carries no dollar amount of its own. The money moves under the group code and reason code on the same line. M51 tells you which field to fix: the CPT or HCPCS code in Item 24D, or loop 2400 on an electronic claim.
What M51 means in plain English
Each part of the line answers a different question:
- The group code (usually CO) says who carries the amount. CMS describes CO adjustments as amounts “generally… considered a write off for the provider and are not billed to the patient” (Medicare Claims Processing Manual, Ch. 22, §60.1).
- The reason code (usually 16) says the claim “lacks information or has submission/billing error(s).”
- M51 says the error is in the procedure code.
Missing, incomplete or invalid covers more than typos. For example, a code that was valid last year, a code Medicare doesn’t recognize, an unlisted code with no description, or a code that doesn’t match the documented service can all return M51.
Which reason codes come with M51
| CARC + M51 | What it means | Published by |
|---|---|---|
| 16 + M51 (often with N56) | Required claim information is missing or incorrect. Unprocessable; send a new claim | Noridian JE Part B, First Coast, WPS |
| 16 + M51 + N350 | The narrative for a not otherwise classified (NOC) or unlisted code is missing or incomplete | Noridian JA DME |
| B15 + M51 | An administration fee was billed with no drug in the required time frame, or the “from” date is wrong | Noridian JA DME |
M51 with CO, PR, OA or PI
The group code on the line decides who owes the money:
| Group + CARC + M51 | Who carries the amount | Can you bill the patient? | When you’ll typically see it |
|---|---|---|---|
| CO-16 + M51 | Provider | No | The usual Medicare form. WPS: “You can only correct a rejected or unprocessable claim by submitting a new claim with the correct information.” |
| PR + M51 | Patient | Not for a coding error | Shouldn’t happen for a bad procedure code. Ask the payer to correct the group code |
| OA + M51 | Neither group fits | Check your contract first | Rare. Used “when no other group code applies” (CMS Ch. 22, §60.1) |
| PI + M51 | Payer-initiated reduction | No | Non-Medicare remits only. CMS lists just CO, OA and PR as valid on Medicare remittances |
Remark codes that travel with M51
| RARC | Official X12 text | What it adds |
|---|---|---|
| N56 | “Procedure code billed is not correct/valid for the services billed or the date of service billed.” | The code doesn’t fit the service or the date |
| N350 | “Missing/incomplete/invalid description of service for a Not Otherwise Classified (NOC) code or for an Unlisted/By Report procedure.” | The unlisted or NOC narrative is the problem |
| MA130 | “Your claim contains incomplete and/or invalid information, and no appeal rights are afforded because the claim is unprocessable…” | Medicare’s unprocessable alert. No appeal |
| M20 | “Missing/incomplete/invalid HCPCS.” | The same problem, reported for a HCPCS Level II code |
| N301 | “Missing/incomplete/invalid procedure date(s).” | X12 lists it as related to M51. Check the dates too |

Where M51 shows up in the 835
On an electronic remittance (835), M51 sits in the LQ segment under the service line. Here’s a trimmed, illustrative example for a $650.00 unlisted procedure line:
SVC*HC:64999*650.00*0.00**1~
DTM*472*20260910~
CAS*CO*16*650.00~
LQ*HE*M51~
LQ*HE*MA130~
CAS*CO*16*650.00is the group code, reason code and adjusted amount.LQ*HE*M51names the procedure code as the problem.LQ*HE*MA130is Medicare’s alert that the claim is unprocessable and has no appeal rights.
How to fix M51: decision path

- Read every remark code on the line. A second remark code, such as N56, N350 or N301, narrows the fix.
- Check the code against the date of service. First Coast: “Select a valid procedure code using the most current year’s CPT and/or HCPCS code(s) based on the date(s) of service on your claim.”
- Check Medicare status. First Coast says that if the code “has an “I” status, the procedure code is not valid for Medicare and should not be billed unless documentation is needed for a secondary payer or supplemental plan.” Use your MAC’s fee schedule lookup.
- For unlisted or NOC codes, add the narrative. Noridian: “Enter a brief yet complete description of services rendered in Item 19 of CMS-1500 claim form or SV101-7 field,” and “If characters extend beyond SV101-7, continue entering in Loop 2300 NTE Segment.” Our Box 19 guide for unlisted procedures has examples.
- Make sure the code matches the record. If N56 is present, the code may be valid but wrong for what was done. Recode from the documentation.
- Send a new claim. For Medicare, don’t appeal or reopen. Noridian: “Submit new claim with corrected/complete information.” For other payers, follow their corrected claim rules.
- Watch the filing limit. The new claim must still arrive within the payer’s timely filing window.
Medicare and major payer notes
No appeal rights. Noridian’s CO-16/M51/N56 page says: “Claim is unprocessable; no appeal rights.” WPS adds that you “cannot request a redetermination on these claims because they have not received an initial determination,” and that they “do not qualify for a Clerical Error Reopening (CER).” See our MA130 guide for how unprocessable claims work.
Timely filing still runs. First Coast warns that unprocessable claims “are not considered filed / submitted,” so the corrected claim has to reach Medicare within 12 months of the date of service.
Unlisted and NOC codes. CMS’s Part B data element rules list this exact case: an “unlisted procedure code” or NOC code in Item 24D “but an accompanying narrative is not present in item 19 or on an attachment. RARC: M51” (CMS Ch. 1, §80.3.2.1.1). Noridian adds that for an unclassified drug code you should “enter drug name and dosage in Item 19” and “Enter one (1) unit in Item 24G.”
Medicare DME. Noridian JA DME pairs M51 with N350 when the “Narrative on claim missing information for code of item being provided or repaired.” Its fix is to “Correct/update narrative and resubmit as a new claim.”
Commercial plans. Other payers, such as Medicare Advantage and commercial plans, use M51 the same way, but some reject the claim at the clearinghouse before it ever produces a remit. Check your 277CA rejection report as well as the 835, and follow the plan’s rule on new versus corrected claims.
Worked example (illustrative)
A practice bills Medicare Part B for an unlisted nervous system procedure, CPT 64999, at $650.00. The coder left Item 19 blank and sent no attachment. The remit comes back:
CAS*CO*16*650.00withLQ*HE*M51andLQ*HE*MA130
Reading it: the code itself is valid, but CMS requires a narrative for unlisted codes. Without one, the claim is unprocessable, and MA130 confirms there’s no appeal.
Fix: submit a new claim with a short description of the procedure in Item 19, or in SV101-7 and the 2300 NTE segment on an electronic claim, and send the operative report the way your MAC asks for unlisted procedures. Don’t request a reopening.
Prevention: add a scrubber edit that blocks any unlisted or NOC code when Item 19 or SV101-7 is empty.
M51 corrected claim worksheet
There’s no appeal letter for a Medicare M51, so use this worksheet before you resend. For commercial payers that allow a corrected claim, it doubles as the cover note. Replace the bracketed text.

M51 prevention checklist
- Load each year’s CPT and HCPCS updates into your PM system before January 1, and quarterly HCPCS changes as they’re released.
- Block deleted codes by date of service, not by the date the claim is built.
- Flag codes with Medicare status “I” on Medicare claims.
- Require a narrative for every unlisted and NOC code before the claim can go out.
- Match each code to the documentation during charge review.
- Review clearinghouse rejection reports daily, since some M51 errors never reach an 835.
- Track M51 by provider and code to find training gaps.

FAQ
What is the M51 remark code description?
X12 describes RARC M51 as “Missing/incomplete/invalid procedure code(s).” It points to the CPT or HCPCS code on the line, in Item 24D of the CMS-1500 or loop 2400 of an electronic claim.
What does remark code M51 mean?
It means the payer couldn’t use the procedure code as billed. The code might be missing, deleted, not valid for the date of service, not recognized by the payer, or an unlisted code sent without its narrative.
How do I fix CO-16 with M51?
Check the code against the date of service and the payer’s fee schedule, recode from the record if needed, and add a narrative for unlisted or NOC codes. Then send a new claim. For Medicare, don’t appeal or request a reopening.
Can I appeal an M51 denial from Medicare?
No. CO-16 with M51 makes the claim unprocessable. Noridian says “Claim is unprocessable; no appeal rights.” Send a corrected new claim within the 12-month filing limit instead.
What is the difference between M51 and N56?
M51 says the procedure code is missing, incomplete or invalid. N56 says the code “is not correct/valid for the services billed or the date of service billed.” Together they often mean the code doesn’t fit the service or the date.
Can I bill the patient when M51 appears?
No. The line usually carries CO-16, which is a provider responsibility. Fix the code and resubmit so the payer can process it.
